- Distress (rank 1): 5 Damage, 25 threat for 7 mana.
- Persecute (rank 2): 70 Damage, 50 threat for 85 mana.
Sustain is worse at higher levels. We can get away with spamming a 7 mana Distress at very low levels. By level 15-ish that isn't enough threat generation and we start struggling to hold aggro. The first upgrade we get is level 20, a 19 level gap on our primary threat tool, for 85 mana (300%+ increase in mana per threat.) The efficiency keeps getting worse at each new rank.
If the Distress line was a once per mob debuff, after which we could rely on our melee kit to hold aggro, it'd still be expensive but manageable at 85 mana. Since Purifying Blow and Censuring Strike (which has no threat modifier) generate insufficient threat, we're forced to spam the Distress line several times which is just not feasible beyond rank 1 because of its cost.

Inquisitors will perform much better for a group if they have an enchanter or bard.
Ironic considering the only things of any significant value an Inquisitor brings to a group are mez and haste.
With a Bard or Enchanter providing both of those your group is better off with any other tank given the mana and threat issues limiting Inquisitor at the moment.
Hopefully some more tuning happens before EA.
Xilith has alluded to these issues as being intentional
To be fair, I think he meant that mana users aren't intended to have unlimited mana, even as a tank, which is reasonable. I don't think it's intended for Inquisitors sustain to stay this bad. It's just not tuned properly yet.
